Save Form Results

I have Forms on my project. I'd like to Save the results of whatever is entered in the textboxes and labels on that specific form I'm entering into. The attached Sample Form may help as to what I'm trying to accomplish. I'm not looking for a data base or other means of saving results, just on a specific form.
